We are searching for a talented Social Worker-Clin-Case Coordinator-LICSW *$1,800 new hire sign-on bonus with LICSW or LMHC license at time of hire. Compensation: Pay rate will be determined based on level of experience. Responsibilities Under the direction of the Manager, Clinical Social Work Services; perform diversified duties both within and in support of the SHG Care Coordination functions. Provide case management services including but not limited to the application of clinical diagnostic, assessment or treatment knowledge or skills. Identify and address psychosocial factors that may impede the patients' ability to optimize benefit from the health care delivery experience. Coordinate the care/services of assigned patients along the continuum of care. Participate in discharge planning to assure timely individual patient discharges within the scope of position responsibilities. Facilitate patient/family/representative achievement of desired outcomes. Responsibilities may include on call/shift coverage and holiday coverage. Qualifications
