The Collaborative Drug Therapy Management (CDTM) Neurology Pharmacist partners with collaborating physicians to support the mission of providing exceptional patient care across the Health System. They serve as a liaison between patients, ambulatory, clinical and pharmacy teams, facilitating and optimizing efficient provision of drug therapy and positive patient outcomes. The CDTM Pharmacist provides consulting services to patients regarding the effective use of medications, importance of adherence, awareness of drug interactions, and may coordinate other preventive healthcare services such as immunizations. CDTM Pharmacist plays an essential role in disease state management, identifying ways to optimize medication therapy and ensuring an optimal patient experience.
