Job Summary

The Substance Use Disorder Patient Navigator will provide transitional support to individuals struggling with Opioid Use Disorder who are currently in custody and/or recently released from San Francisco County Jails. The Navigators will enroll and serve 200 patients over the 5 year grant period individually and in group settings with a focus on harm reduction counseling and opioid treatment linkage. Navigators will work in the jails and at the OBIC Clinic, located at 1380 Howard Street. Their ultimate goal will be to connect patients with a provider in the community who can continue their medication treatment for Opioid Use Disorder. Other objectives that Project JUNO Community will focus on will be to decrease the risk of overdose and other substance use related comorbidities, improve physical and mental health, improve employment and housing status, improve social connectedness, for patients up to 6 months after their release. Navigators will also administer standardized questionnaires at intake, 3 months, 6 months and at program discharge.

The final salary and offer components are subject to additional approvals based on UC policy.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.

The salary range for this position is $55,200 - $95,900 (Annual Rate).

Department Description

The mission of the Division of Substance Abuse and Addiction Medicine (DSAAM) is to improve the quality of life for our patients and the public by providing the highest quality addiction treatment, education, and research to reduce the dangers of drug abuse and its consequences. DSAAM provides multi-disciplinary consultation and treatment for patients with substance use disorders at San Francisco General Hospital (SFGH) and several affiliated clinics (including OBIC Clinic). The main clinics include a licensed opioid treatment program (Ward 93/OTOP), the nation's first buprenorphine induction clinic (OBIC Clinic), mobile methadone vans, and a linkage program (HOUDINI LINK).

Project JUNO Community staff will come from DSAAM in the UCSF Department of Psychiatry and Behavioral Sciences which has over 40 years of serving the population of focus and providing comprehensive care management to complex, underserved populations. DSAAM will also partner with Jail Health (Tyler to insert description of services here). Project staff are highly trained individuals who are state and nationally recognized leaders in the treatment of OUD, including in the correctional setting. Referrals to residential treatment, primary care and treatment of mental illness will also be available to provide a continuum of care individualized to meet patients' needs.

Required Qualifications

• Bachelor's degree in related area and minimum three years of experience / or equivalent experience / training.
• Strong familiarity with community resources, organizations, and harm reduction practices

• Demonstrated excellent attendance and reliability.
• Nine hour law/ethics and confidentiality online course to be completed through CCAAP upon being hired.

Preferred Qualifications

• Training and experience in delivering behavioral health interventions to persons with substance use disorders as well as conducting point of care urine drug screens with fidelity to established policies and procedures.
• In-depth knowledge of the organization and how to get issues resolved.
• Proficiency with Windows-based software including Microsoft word, Excel, Outlook. Knowledge of computer systems and software used in functional area.
• Familiarity with various supportive counseling strategies (Motivational Interviewing, Wellness & Recovery Principles, Harm Reduction, Trauma informed care, and more)
• Strong knowledge of Patient Rights & Responsibilities, Joint Commission standards, and Centers for Medicare / Medicaid regulations. Knowledge of Medical Terminology. Strong knowledge of data collection, compilation, and analytical techniques.
• Ability to speak, write, and understand Spanish.
• Registration with a California approved Substance Abuse Counselor Certification Organization (http://www.dhcs.ca.gov/provgovpart/Pages/CounselorCertificationOrganizations.aspx
• Completed counselor certification may be provided in lieu of counselor registration
