Responsibilities

* Collaborates with physicians, medical team members, and families to develop and implement comprehensive care plans, ensuring patients receive appropriate medical and psychosocial care.
* Provides personalized navigation support to patients and families, helping them move through the healthcare system and addressing immediate needs in real time.
* Connects families to resources that address social determinants of health and accommodate specific cultural and linguistic needs.
* Assists patients and families with scheduling multidisciplinary appointments, coordinating logistics such as appointment reminders, transportation, and housing.
* Manages administrative functions, including preparing and maintaining records and case files, obtaining prior authorizations, and supporting program-related data collection and management.
* Serves as the primary point of contact for patients and families, answering incoming calls, scheduling appointments, and providing frontline support for patient questions with referrals to appropriate care providers.
* Monitors and manage transitions between settings, caregivers, and providers, providing follow-up across the continuum of care and participating in care planning meetings.



Qualifications

Education: Bachelor's Degree in health care, public health, social work, or related clinical field, or equivalent education and experience, required.

Certifications: Active BLS certification, preferred.

Skills:
* Effective verbal and written communication and interpersonal skills.
* Effective analytical, problem solving, and critical thinking skills.
* Excellent time management and prioritization skills.
* Attention to detail needed to operate accurately and effectively.
* Proficiency with MS Office and electronic medical record documentation and/or reporting.
* Comfort with working with special needs patients.

Experience:
* Two years of experience in healthcare or medical research, preferred.
* Experience working with people with Williams syndrome or Down syndrome, preferred.
