Overview Provides personalized navigation services to patients and families in the Williams Syndrome and Down Syndrome Programs, ensuring seamless access to medical and psychosocial care by coordinating appointments, managing transitions, and connecting them to essential resources. Responsibilities * Collaborates with physicians, medical team members, and families to develop and implement comprehensive care plans, ensuring patients receive appropriate medical and psychosocial care. Qualifications Education: Bachelor's Degree in health care, public health, social work, or related clinical field, or equivalent education and experience, required.
