12-Hour Rotating Shifts

No Direct Bedside Patient Care

JOB SUMMARY:

We are seeking a dedicated Nurse Navigator RN to help guide patients through the healthcare continuum, while supporting populations across the country. This role offers the opportunity to use your clinical expertise in a collaborative, technology-enabled environment without providing direct bedside care. As a Nurse Navigator RN, you will coordinate care, support informed decision-making, and help patients navigate complex healthcare needs while serving as an important member of our integrated care team.

ROLES AND RESPONSIBILITIES

Be a Catalyst for Change

  • Utilize clinical expertise to connect patients with appropriate healthcare resources and services.

  • Support innovative care delivery models that improve patient outcomes and healthcare access.

Clinical Decision Support

  • Conduct telephonic nursing assessments and clinical evaluations using evidence-based clinical decision support tools and approved protocols.

  • Identify patient needs and facilitate appropriate disposition and care navigation recommendations.

Patient Education & Advocacy

  • Educate and empower patients regarding healthcare options, chronic disease management, preventive care, and healthy behaviors.

  • Advocate for patients to ensure access to appropriate healthcare resources.

Technology and Innovation

  • Utilize telehealth, telemonitoring, and care coordination platforms to support patient engagement and healthcare delivery.

  • Document interactions accurately and efficiently within established systems.

Collaboration & Preceptorship

  • Collaborate with multidisciplinary team members to support patient care coordination.

  • Participate in quality improvement initiatives, training, and program development activities.

  • Support onboarding, mentoring, and education efforts as needed.

Operational Excellence

  • Maintain productivity, documentation, and quality standards.

  • Provide exceptional customer service while managing multiple priorities in a fast-paced environment.

  • Represent GMR's mission, values, and innovative healthcare programs professionally.

Other duties as assigned.

Minimum Required Qualifications

  • Graduate of an accredited nursing program.

  • Active Registered Nurse (RN) license in a Nurse Licensure Compact (NLC) state of Residence.

  • Minimum of three (3) years of RN experience in Emergency Department, Acute Care, Critical Care, Urgent Care, Telehealth, Nurse Triage, or a similarly fast-paced clinical environment.

  • Current CPR certification.

  • Experience in patient advocacy, clinical documentation, and healthcare navigation.

  • Strong critical thinking, clinical judgment, communication, and customer service skills.

  • Ability to effectively utilize technology, electronic medical records, and clinical decision-support systems.

Multi-State Licensure Requirement

  • Must possess and maintain an active state of residence Compact RN license in good standing.

  • Must be willing and able to obtain and maintain additional RN licensure in non-compact states as required by business needs and operational demands.

  • Ability to successfully secure and maintain licensure in multiple states within employer-established timelines is a condition of employment.

  • Participation in employer-sponsored licensure support and reimbursement programs may be required, where applicable.

Preferred Qualifications

  • Bachelor's degree in nursing (BSN)

  • Experience in care coordination, population health, case management, telehealth, nurse triage, or community health programs.

  • Experience working with clinical decision-support protocols and telephone nursing services.

  • Telephonic Triage experience.
