Medical Center: Community Health Partnership- (Full-Time, Day Shift) - Job Summary: The Director- Community Health-Grant oversees community health projects including Community Health Worker (CHW) integration, grant proposal writing, management, and reporting. Directs and manages the implementation and evaluation of community health programs, such as convening partners, leading initiatives, and sharing results with institutional leadership and external stakeholders. Performs other duties as needed. Education and Experience: Bachelor's Degree in Public Health, Behavioral Health, or Business Administration required. Minimum of five years of experience working with cross-sectoral collaborations. Knowledge and Skills: Knowledge of social drivers of health, program planning and evaluation, grant administration, budget oversight, quality improvement methods, and applicable healthcare, privacy, compliance, and safety regulations. Working knowledge of community partnership development, stakeholder engagement, data reporting, and operational procedures. Proficient use of standard office technology and software, including word processing, spreadsheets, presentation tools, email, virtual meeting platforms, and data tracking systems. Ability to prepare reports, analyze program and budget data, develop presentations, review contracts and grant materials, and travel locally for meetings and community events. Relate and communicate positively, effectively, and professionally with others; provide leadership; be assertive and consistent in enforcing policies ensuring compliance with regulatory standards, rules and laws; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Think critically; work independently; perform basic math and statistical functions; manage multiple assignments; compose written material; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail. Distinguish colors as necessary; hear sufficiently for general conversation in person and on the telephone, and identify and distinguish various sounds associated with the workplace; see adequately to read computer screens, and written documents necessary to the position.
