JOB RESPONSIBILITIES: 1. Conduct Wellness Exams as assigned. 2. Assist with triage and telephonic needs of patients. 3. Assist in smooth workflow of clinic by: - Assist on clinical floor as needed. - Assist with emergent and acute patients. - Execute nursing interventions through collaborative work with Providers and support staff. - Provide care coordination, develop treatment plans, and assist with the development of patient self-management goals for complex adult patients not covered by other programs or insurances. - Connect patients with case management services provided through their coverages. - Evaluate readiness and progress towards goals for complex patients as assigned and communicate such with Providers and support staff. - Provide education, information, direction and support related care goals. - Advocate for and assist with problem solving and addressing barriers to care. - Troubleshoot problems regarding operational and clinical procedures that may affect patient outcomes. - Provide notification of normal and low acuity test results (limited to FIT, STD, mammogram and pap results and radiological studies). 4. Assist in maintenance/protection of patient privacy and medical record data collection by: - Proactively upholding clinic guidelines and standards related to Patient Confidentiality. - Review patient data on chart each visit for necessary health maintenance issues and/or follow-up issues from previous visits. - Completing accurate and professional documentation of care rendered, procedures performed, tests administered, tasks completed, results received, and/or other patient related information pertinent to care. 5. Participate in the Clinic mission to provide high quality healthcare by: - Actively participate in department In-servicing and Annual Skills Testing. - Actively seek independent studies to keep aware and current of nursing issues. - Alert appropriate staff and/or acting on possible safety or confidentiality issues. - Offer workflow, safety, morale related improvement suggestions. 6. Perform specialized clinical procedures (Synagis injections, catheterizations, IV insertions, phlebotomy, Stress testing, NSTs) as needed. 7. Proactively participate in staff development by mentoring, seeking to provide "on-the-spot" learning opportunities, creating in-servicing plans, delivering in-servicing plans. 8. Comply with SSCHC Infection Control Plan including employee testing. 9. Maintain additional assigned clinical projects (staff development, Coumadin clinic, other administrative duties) in a safe, orderly, and documented manner, with minimal supervision requirements. QUALIFICATIONS: 1. Registered Nurse with current license and two years nursing experience; current basic life support certification (CPR). 2. Supervisory experience or demonstrated supervisory aptitude. 3. Strong computer skills preferred. 4. Experience in community health, public health or in a community agency. 5. Ability to relate well to people from diverse ethnic and cultural backgrounds. 6. Bilingual preferred (Spanish/English or Hmong/English). | |
