About Rivendell:

Rivendell is a 149 inpatient psychiatric hospital located in the heart of Bowling Green, KY on a 49 acre campus. At Rivendell Behavioral Health Hospital, children who are in crisis receive psychiatric care. Teens suffering from depression, disabling anxiety, feelings of hopelessness or suicide can learn new ways to cope. Teens and adults suffering from chemical dependency and substance abuse can get help.

POSITION SUMMARY

Coordinates and tracks all of the discharges in the hospital. This includes maintaining of an ongoing
discharge calendar, aftercare providers, and trends regarding patient discharges. Works closely with
Case Management Supervisor to carry out objectives regarding the provision of utilization
management, discharge planning, clinical care and payor source coordination for patients.


Qualifications

QUALIFICATIONS

Education:
A Masters degree in Social work, Psychology, or other Mental Health field is preferred.
A BA degree in Mental Health or Human Services related field or a Bachelor's degree with 2 years
experience in a Psych Hospital or a degree in Nursing and licensure by Kentucky as a Registered
Nurse or Licensed Practical Nurse is required.

MAJOR DUTIES AND RESPONSIBILITIES
1. Maintains an ongoing and accurate record of all scheduled discharges in the hospital.
2. Communicate with the therapist/case managers to anticipate and report any foreseeable
problems regarding the continued stay or discharge planning of patients.
3. Monitor and report to Case Management Supervisor the ongoing documentation regarding
discharge planning to help identify problems given that interventions can proceed issues
which hinder discharge planning.
4. Maintains ongoing list of referral sources and aftercare providers for all patients.

5. Coordinates with the therapist, physician and other treatment team members, to ensure that
referral sources, aftercare providers, and other involved parties are communicated with
appropriately.
6. Attends treatment team meetings on all units as needed to provide and collect information
pertinent to the continued stay and discharge planning of patients.
7. Completes reviews of charts and makes recommendations to therapist and treatment team
regarding balancing of resources with discharge planning.
8. Contacts reimbursement agencies at regularly identified intervals to justify admission to
hospital, continued hospitalization and discharge planning.
9. Completion of Discharge Planning and aftercare arrangements for all assigned patients. This
includes scheduling of aftercare appointments, and coordinates discharge conference.
10. Will adhere to all safety policies and safe work practice.
11. Will attend hospital safety trainings.
12. Will adhere to all hospital policies and procedures.
13. Will report all accidents immediately.
