SUMMARY The Behavioral Health Specialist is a member of the Center for Population Health's Enterprise Ambulatory Care Management team and is accountable for improving the health outcomes of the populations being managed. They work collaboratively with NEMG/YIMA Primary Care, Ambulatory Care Management teams, and healthcare professionals across the Yale New Haven Health System (YNHHS). This role is responsible for the development, implementation, coordination, and evaluation of behavioral health services for patients across the continuum of care. The Behavioral Health Specialist is a licensed, experienced clinician who serves as a subject matter expert in the assessment, treatment, and coordination of behavioral health services across YNHHS delivery networks. Qualifications EDUCATION: Masters degree in Social Work or a related behavioral health field from an accredited institution. EXPERIENCE: Minimum of five (5) years of clinical experience addressing behavioral health needs across the spectrum of care SPECIAL SKILLS:
LICENSURE/CERTIFICATION: LCSW and or related Behavioral Health License to practice in CT PHYSICAL DEMAND MMR is required. Varivax (chicken pox vaccine) or evidence of prior chicken pox is required. Hepatitis B (or signed declination) is required for those with potential exposure to blood/body fluids. Tdap strongly encouraged and influenza vaccination required.
