General Summary of Position

MedStar Health is looking foranLCSW-C/LICSW Inpatient Case Managerto join our team at Washington Hospital Center!

The LCSW-C/LICSWLICSWservesas a member of the Case Management Team. Facilitates the delivery of quality, cost effective, patient-centered care from pre-admission through post-dischargetimeframe. Ensures the care is designed to meet individualized patient outcomes. Monitors the care and services delivered to selected patient populations during the acute hospital stay working collaboratively with the multidisciplinary team both internal and external to the organization, promotes effective case management and utilization of resources facilitating the continuum of care, and works to achieve optimal clinical and resource outcomes for the acute, and posthospital phases of care.

Primary Duties:

  • Collaborates with the multidisciplinary healthcare team to assess patient needs, develop and coordinate individualized plans of care, andfacilitatesafe,timelydischarges.

  • Conducts psychosocial and socioeconomic assessments, provides clinical evaluations for behavioral health concerns, and connects patients withappropriate communityand social service resources

  • Facilitates communication between patients, families, and healthcare providers to address social, emotional, cultural, and financial barriers tocare..

  • Documentsassessments, referrals, and interventions accurately whilemaintainingcompliance with departmental policies, quality standards, and regulatory requirements.

  • Participates in quality improvement initiatives, meetings, and professional development to enhance caremanagement effectiveness and departmental performance.

Qualifications:

  • Master's degree in social workfrom a school accredited by the Council on Social Workeducation(Required)

  • 1-2yearsExperiencein social work,preferablein a hospital setting(Preferred)

  • 1-2yearsExperiencein care/case(Preferred)

License/Certifications:

  • LCSW-C - Licensed Certified Social Worker -Clinical Licensein the District of Columbia or the State of Maryland depending on work location(Required)

  • LICSW - Licensed Independent Clinical Social WorkerLicense in the District of Columbia or the State of Maryland depending on work location(Preferred)

  • CCM - Certified Case Manager(Preferred)
