Job Description:

Johns Hopkins Hospital is seeking a dedicated and experienced Nurse Practitioner to join its Palliative Medicine team in a primarily outpatient capacity. This role supports the expansion of outpatient palliative care services and offers an opportunity to provide comprehensive, patientcentered care within a highly collaborative academic medical environment.

This position is intended for clinicians who are actively and intentionally pursuing a career in palliative care. Candidates must demonstrate a strong alignment with palliative care philosophy and the ability to practice autonomously in an outpatient setting.

Key Responsibilities:



  • Conduct comprehensive palliative care assessments and develop individualized care plans.
  • Provide symptom management, goalsofcare discussions, advance care planning, and psychosocial support for patients with serious illness.
  • Manage a panel of patients independently in both stand-alone and embedded outpatient clinic settings.
  • Collaborate closely with an interdisciplinary team, including physicians, social workers, nurses, pharmacists, and chaplaincy.
  • Coordinate care with specialty services (e.g., oncology, cardiology, neurology) and community partners, including hospice agencies.
  • Communicate effectively with patients, families, and referring providers to support shared decisionmaking.
  • Participate in interdisciplinary meetings, case discussions, and quality improvement initiatives.



Schedule:




  • Monday-Friday, fulltime
  • Majority of clinical time will be based on the East Baltimore / JHH main campus
  • Primarily outpatient
  • No call



Qualifications:




  • Master's degree in Nursing from an accredited program.
  • Current Maryland Nurse Practitioner license.
  • Certification as a Nurse Practitioner in a relevant specialty
  • Strong clinical assessment skills and experience in palliative care, oncology, or geriatrics preferred.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively within a multidisciplinary team

Salary Range: Minimum 53.15/hour - Maximum 82.38/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
