Description
The Patient Flow Provider (PFP) is a full time position (1.0 FTE). The primary purpose of this role is to facilitate timely discharges. The current schedule is 5 days per week (Mon-Fri) from 8am-4pm. Patients on the Hospital Medicine teams with low acuity and short lengths of stay are the targeted population. The PFP does not carry their own patients, but rather works in parallel during the rounding period in the morning to expedite the discharges in close communication with the respective team's providers. Afternoon time is focused on anticipatory discharge planning for those patients who are close to meeting discharge criteria in the coming 8-12 hours. The Advanced Practice Provider (APP) is credential by the medical staff and functions within their scope of practice in their jurisdictional area and in accordance with organizational medical staff bylaws. They are responsible for providing evidenced-based care for the management of patients, including assessment, implementation and evaluation of a detailed plan of care, which incorporates diagnostic and therapeutic interventions. The APP demonstrates competency in providing age-appropriate care to patients as defined by certification and training and provides family centered care to meet family needs, including psychological support, education and discharge planning in conjunction with other related disciplines.
