Description Maine Medical CenterNursing Summary: The Care Management Education Specialist is responsible for developing, implementing, and evaluating education programs that support clinical care managers, social workers, utilization review staff, and interdisciplinary care teams. This role strengthens care management competencies, ensures compliance with regulatory requirements, promotes best practices in care coordination, and supports organizational goals related to quality, patient flow, and readmission reduction.Special attention to the Post acute care community education is required. This role also requires an innovative and adaptable individual who can enhance team performance through creative problemsolving, collaboration, and continuous improvement. The position follows a Monday-Friday schedule, with the expectation that the incumbent may be pulled to support care management assignments as needed to ensure optimal patient flow and team coverage.
