Come join our team and love what you do! We are currently hiring anOutpatient Therapist Assessment/Milieu Coordinator for Fremont Hospital! The Therapist Assessment/Milieu Coordinator performs intake assessments and determines medical necessity/level of care for patients admitting to the outpatient program. The Assessment/Milieu Coordinator Therapist also supports managing the outpatient milieu by reviewing daily attendance of patients, responding to patient crises and performing daily safety/wellness checks for patients within the program. The Assessment/Milieu Coordinator Therapist performs case management duties including the assessment, discharge planning, reporting during treatment planning, maintaining patient attendance to the prescribed treatment, ensuring documentation compliance, and following up to ensure the safety of all patients. The Assessment/Milieu Coordinator Therapist coordinates the initiation of a Master Treatment Plan and ensures updates are consistent with the findings of initial assessments, ongoing assessments, treatment team meetings, and other documented observations. The Assessment/Milieu Coordinator Therapist attends daily utilization review/clinical meetings and ensures the behavioral treatment provided to the patient is consistent with what is communicated to and understood by the Utilization Review Specialist, management and other team members; this information is documented consistently in the chart across disciplines. The Assessment/Milieu Coordinator Therapist facilitates groups and 1:1 meetings using an evidence-based treatment consistent with research findings. The Assessment/Milieu Coordinator Therapist exercises sound judgment, and clinical excellence sustains therapeutic alliance and patient satisfaction. The Assessment/Milieu Coordinator Therapist will be cross-trained and assigned additional duties, as determined by the Intake and Utilization Review departments. Experience: A minimum two (2) years of direct clinical experience on a multidisciplinary team in a psychiatric, mental health setting or comparable. Experienced in patient assessment, family motivation, treatment planning and communication with external review organizations or comparable entities. Ability to provide skills training and cognitive restructuring within the cognitive-affective-behavioral mode used and other evidence-based treatment, CBT and DBT experience preferred. Licensure: LPCC, LMFT, LCSW, Licensed Psychologist, or PsyD required. Certification(s): CPR/AED Certification must be current and presented to Human Resources upon hire. You may be required to successfully complete Fremont Hospital's non- violent crisis prevention intervention training course within 30 days of employment and prior to assisting with preventative intervention measures.
