The Case Manager is responsible for providing case management to participants. Provide advocacy, support, and referral information and assist individuals or families to overcome barriers to become self-sufficient.

Essential Duties & Responsibilities:

Complete intake and needs assessments with each participant, which may involve a broad range of areas including housing, finance & budgeting, employment, education, food & nutrition, school & daycare, legal issues, health, parenting, and daily living skills.

Interview and evaluate clients to formulate Individualized Service Plans based on Program requirements. Work closely with clients to identify and build upon strengths and develop strategies to address barriers and concerns identified through the assessment process.

Help clients build self-sufficiency plans. Assist clients to transition to self-advocacy roles.

Advocate on behalf of clients and attend administrative hearings if necessary.

May perform home visits and/or unit inspections.

Establish, evaluate and monitor goals for clients linked to measurable outcomes. Work collaboratively with interdisciplinary team of collateral providers and partners to ensure coordination of services.

Maintain the structure and policies of the program through orientation of participants by reviewing program rules, policies, and case management visits.

Maintain accurate case records by documenting all services rendered including up to date case notes, telephone contact logs and referrals. Enter required data into the agency databases with reporting as required.

Provide emergency response management to address any crises or safety issues that may arise.

Accompany and/or transport participants to appointments as necessary. Assist families with arranging appointments.

On-call duties as assigned.

Qualifications:

High School Diploma or equivalent required. Bachelor's Degree in related field preferred.

Minimum of one-year assessment, advocacy and case management work in a Human Service field required.

Knowledge and experience working with low income, culturally diverse, and at-risk families and individuals.

Demonstrated knowledge of community resources available to low-income individuals and families.

Excellent customer service, organizational, oral and written communication skills.

Ability to manage multiple priorities and work in a fast-paced environment.

Ability to work and travel between Bedford, MA and Lowell, MA locations.

Must have reliable transportation.

Proficient computer skills required in basic computer software and data collection systems.

Excellent crisis management skills.

Ability to maintain accurate records and confidentiality of information.

Excellent attention to detail.

Valid driver's license.

Bilingual English/Spanish strongly preferred.
