CLTS Case Manager

Washington County, located in West Bend, Wisconsin has an excellent opportunity for a Case Manager - Community Treatment Services. This position is responsible for clients diagnosed with mental health and or substance use involved with outpatient care, targeted case management, Community Support Program, and the Comprehensive Community Services program.
Qualified candidates must possess:

*Bachelor's Degree preferred in social work, human services, or related field.
*One year of supervised work experience
*Knowledge of local community resources for clients
*Knowledge of medications preferred
*A valid Wisconsin driver's license, available automobile, and automobile insurance are required.

Pay range is: $29.64-$39.40

Position Summary:

Under the general direction of the Community Treatment Services Supervisor, serves as case coordinator for clients diagnosed with mental health and or substance use involved with

outpatient care, targeted case management, Community Support Program, and the Comprehensive Community Services program.

Duties/Responsibilities:

The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.

*Coordinates with community resources, refers clients for services, and monitors client involvement in services.

*Understands all relevant statutes applicable to job.

*Participates in administrative and clinical meetings.

*Performs routine clinical assessments, including intake interviews of new referrals.

*Provides support and education to clients, family members, and other natural supports.

*Develops, directs and implements client treatment plans, behavioral approaches, and skill training.

*Serves as an advocate for clients and families.

*Acts as a facilitator or co-facilitator for individual and group treatment within or outside of the agency.

*Prepare, complete and submit required items by due dates such as progress notes, service plans, assessments, and court required documents, etc.

*Distributes medication to clients in the community, when necessary.

*Coordinates with crisis and chapter 51 liaison regarding commitment status and crisis needs

*Acts as a mandated reporter under current policies and procedures.

*Participates in continuing educational programs.

*Performs other duties as assigned, including responding to an emergency event.

Work Direction Received:

Works under the general direction of the Community Treatment Services Supervisor.

Supervision Exercised:
None.

Decision Making:
Decisions concerning crisis intervention, the focus and direction of treatment, and daily work priorities are made independently.
Interaction:

There is significant interaction with area health care providers, Family Care, Social Security Administration, other County Departments, clients and their families. May interact with local law enforcement agencies.

Essential Knowledge and Abilities:

Knowledge of local community resources available for clients; applicable State, Federal and local laws, rules and regulations affecting clients; and effective treatment protocols. Ability to work effectively with a treatment team, communicate effectively orally and in writing, and interact effectively with area health care providers, clients, and their families. Able to fulfill physical demands of job, sensory demands (seeing and hearing), and cognitive demands (concentration, conceptualization, memorization).

Training and Experience:
A bachelor's degree in social work, psychology, human services, or related field, and one year of supervised work experience. Must possess a valid Wisconsin driver's license. Knowledge of medications preferred.
