GENERAL SUMMARY: The Nurse Practitioner or Physician Assistant serves as a first-line provider within a collaborative, multidisciplinary team, delivering inpatient and outpatient care to advanced heart failure and LVAD patients. Responsibilities include management of the Advanced Heart Failure (AHF) clinic, inpatient rounding with the AHF team, consultation for LVAD readmissions, medication management, device interrogations, and urgent clinic visits for VAD patients. The APP provides comprehensive patient and staff education, establishes and coordinates outpatient follow-up plans, and performs clinical assessments, diagnoses, and treatments consistent with credentialing, training, and experience. The role requires close collaboration with attending physicians, advanced practice providers, nurses, consultants, and other members of the care team to ensure safe, efficient, and high-quality patient outcomes.
