Description

Physician Affiliate Group of New York (PAGNY)has an opportunity for a part time Family Medicine Physician - Health Equity for Adolescents in the Legal System (HEAL) with the Administration for Children's Services (ACS).PAGNY staffs several municipal hospitals and clinics within New York City through an affiliation agreement withNYC Health + Hospitals. PAGNY's corporate staff provides administrative, practice management, and human resources services in support of its more than 4,000 clinical providers.

The HEAL Program is a new NYC Health + Hospitalsinitiative to transition into being the primary provider of medical services to young people in City custody. Thestaff will be responsible for 24/7 medical care across two secure detention sites, two school-based health centers, and limited pharmacy services at seven non-secure sites (group homes). Services include comprehensive medical screenings, dental screenings and procedures, vaccinations and routine testing, responses to immediate need, and referrals for offsite care within and outside the NYC Health + Hospitals system.

Opportunity Details:

  • Work locations = Crossroads Juvenile Center and/or Horizon Juvenile Center.
  • Schedule = 21hours per week - Day, Overnight, and Weekend.
  • Provide medical care to ACS adolescents keeping safety andsecurity measures paramount.
  • Takerelevant patient history, identifypatient needs, and makediagnoses.
  • Developand implement treatment plans using diagnostic data.
  • Performcomplete well-care examinations, including physical examinations.
  • Performcomplete gynecological histories and physical examinations.
  • Writeprescriptions and inform patients about medications prescribed; garner parent/guardian consents when needed.
  • Completemedical forms and documentation related to patient care.
  • Makeand track all referrals to other agencies and providers, with follow-up care provided.
  • Assistwith phlebotomy, as needed, to ensure smooth patient flow.
  • Administerimmunizations, as needed.
  • Distribute, refill, discard, and discontinuemedication for patients.
  • CompleteCourt Orders and all other ACS mandated paperwork.
  • Evaluateand treat ACS employees (ONLY on an emergent basis).
  • Provide health education on topics including, but not limited to, preventive medicine and counseling on chronic diseases, as well as parenting, prenatal, nutrition, family-planning, medication usage, and stress management.
  • Administrative duties:
    • Participate in patient outreach.
    • Participatein case conferences in which patient care is discussed.
    • Assistthe Medical Director in the development and implementation of medical protocols and medical forms.
    • Mayinitiate and participate in research related to health care or allied fields, in collaboration with the Medical Director.
Qualifications
  • Current New York Statemedical license.
  • BoardCertification in Family Medicine.
  • Minimumthree years related experience in family, pediatric, adolescent urban populations.
  • Must pass all appropriate screenings to work in a secured setting with young people.
  • Spanishlanguage proficiency preferred (Not Required).
