Case Manager - Crisis Stabilization Unit (CSU) Heritage Behavioral Health Center Heritage Behavioral Health Center is seeking a dedicated Case Manager to join our Crisis Stabilization Unit (CSU) team. This position provides case management and support services to individuals experiencing acute behavioral health crises who require short-term stabilization and coordination of care. The Case Manager plays a key role in ensuring individuals are linked to appropriate community-based services upon discharge and receive the support needed to promote safety, recovery, and ongoing stability. Core Responsibilities:
- Conduct assessments and assist in the development of individualized stabilization and discharge plans.
- Coordinate care and ensure timely linkages to outpatient treatment, housing, medical, and social support services.
- Provide skill-building interventions to enhance coping, problem-solving, and independent living skills.
- Participate in interdisciplinary clinical staffing's to support individualized care planning.
- Provide support and education to individuals and families regarding treatment options and available community resources.
- Monitor individuals' progress and follow up to promote engagement in ongoing services post-discharge.
- Maintain accurate, timely, and thorough documentation in the electronic health record in compliance with agency and regulatory standards.
- Collaborate with internal and external providers, including hospitals, crisis responders, and residential programs, to facilitate smooth transitions of care.
- Support the CSU team in maintaining a trauma-informed, recovery-oriented, and person-centered environment.
Knowledge, Skills, and Abilities:
- Strong ability to engage individuals in crisis and establish rapport quickly.
- Knowledge of behavioral health diagnoses and the use of the DSM-5.
- Understanding of crisis intervention, stabilization, and recovery-oriented practices.
- Familiarity with case management principles, discharge planning, and coordination of care.
- Knowledge of community resources, including psychiatric, substance use, housing, and social service systems.
- Excellent oral and written communication skills.
- Ability to function effectively in a fast-paced, team-based environment.
- Strong organizational and problem-solving skills.
- Flexibility to adapt to shifting priorities and needs within the unit.
- Proficiency in electronic health records and Microsoft Office applications.
Education and Experience:
- Bachelor's or Master's degree in counseling, social work, psychology, or a related human services field.
- Licensure or certification (LSW, LCSW, LPC, LCPC, or CADC) preferred.
- Experience providing behavioral health services in a crisis, inpatient, or emergency setting strongly preferred.
- A valid driver's license, reliable transportation, and proof of current automobile insurance are required.
Salary:
- Bachelor's Degree (MHP): $23.00-$26.00/hour
- Master's Degree (QMHP): $60,000-$67,000/annually based on certification and/or licensure
