Responsibilities:

Make a difference in the health of pregnant and postpartum people and children as a community health worker. The Perinatal Community Health Worker isembedded in Presbyterian Espanola Hospital, providingconnections to community resources, basic health education, and access to care navigation. The Perinatal Community Health Worker will also lead our Espanola produce Rx program for perinatal and children. Qualified candidates will have experience working in community health and/or clinical settings, an interest in the health and wellbeing of their communities, and comfort working with Microsoft Office Suite and using (or learning) an electronic medical record.

This role is expected to work primarily in person in the clinic, as well as some community outreach in Espanola and surrounding areas. The role is part ofa collaborative, statewide Presbyterian Community Health team. Meetings are often held remotely, and occasional travel is required for team meetings and trainings.

Some key responsibilities include:

  • Presbyterian Community Health is seeking a Perinatal Community Health Worker: a dynamic frontline support role responsible for following up on clinical and community referrals for pregnant and postpartum people, providing navigation to appointments and resources, and assisting with Medicaid enrollment. The role leads our produce Rx program in Espanola, enrolling eligible participants and coordinating a weekly food site providing nutritious, local food bags. This role also fosters community partnerships and attends some community events to identify pregnant people who may need help getting into healthcare. The Perinatal Community Health Worker follows guidance and standard operating procedures, documents referral outcomes in both Excel Spreadsheets and EPIC electronic medical record, and provides input and feedback on our approaches.

Qualifications:

  • High school plus specialized, short-term training in clinical, medical skills.

  • Must be able to speak language of the population served.

  • Education Essential: High School Diploma or GED
