POSITION SUMMARY Provides direct psychological, social, and practical care to patients and families, including assessments, problem identification, goal setting, and care plan implementation. Coordinates referrals and continuity of care, collaborates with the multidisciplinary team, and serves as a clinical resource in a specialty area. Provides oversight in the absence of the Clinical Social Worker III/Director, with focus on clinical practice, education, or research. Functions as a mentor and may provide on-call coverage as needed.

Work Experience
Salary Range: Minimum /hour - Maximum /hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
