Youth Services Coordinator - Foster Care & Social Determinants of Health (SDoH)

Location: Indiana (Statewide travel required)

Position Summary

The Youth Services Coordinator will play a critical role in supporting children and youth in Indiana's foster care system. This individual will help young people and their caregivers navigate healthcare services, connect with community resources, and address socio-economic challenges that impact long-term stability and wellbeing. The coordinator will collaborate closely with Community Health Workers (CHWs) across the state to ensure youth and families have access to the right care, housing, food, education, and workforce development opportunities. This position requires regular statewide travel. Candidates must have access to a personal vehicle (mileage reimbursement provided).

Key Responsibilities

* Serve as a primary point of contact for foster youth and their caregivers to connect them with healthcare, social, and community-based services. * Support youth in navigating healthcare systems, including Medicaid and behavioral health services. * Conduct SDoH assessments to identify barriers such as housing, food security, transportation, and financial strain. * Develop and maintain resource networks with community partners, state agencies, and nonprofit organizations. * Work with CHWs statewide to coordinate services and ensure continuity of care for youth. * Provide care coordination and follow-up support to ensure that referrals result in successful connections. * Advocate for youth and families, helping them understand and access their rights and available benefits. * Collect and track program data, documenting outcomes to support program improvement and reporting requirements.

* Promote a safe, supportive, and culturally competent environment for youth and families.

Qualifications Required:

* Bachelor's degree in Social Work, Human Services, Public Health, or related field OR equivalent combination of education and experience.

Experience working with youth, foster care systems, or vulnerable populations. * Knowledge of community resources and SDoH-related services (housing, food, employment, healthcare). * Strong interpersonal, communication, and advocacy skills. * Ability to travel statewide; must have a valid Indiana driver's license, reliable personal vehicle, and auto insurance. * Proficiency with Microsoft Office Suite and case management/documentation systems. * Ability to pass a drug test and background check. * FLU and COVID Immunization.

Preferred: * Community Health Worker (CHW) Certification (or willingness to obtain within 6 months). * Experience in care coordination, case management, or health navigation. * Knowledge of Medicaid, foster care systems, and Indiana community resources. * Bilingual or multilingual skills.

Work Environment & Expectations

* Hybrid role: Combination of in-person community work, travel, and remote administrative tasks. * Travel across Indiana required; mileage reimbursed at the federal rate. * Flexible schedule required to accommodate youth and family needs.
