Performs a variety of tasks that include assisting and monitoring the completion of patient surveys and clinical data entered into the Patient Registry. This position is responsible for patient flow through the survey room, patient data entry and completion of required survey data at critical time points during the patient care continuum. Once entered, this position is also responsible for the integrity of collected data for the purpose of clinical care and research.

I. Major Responsibilities:

1. Assists patients in the direct entry of survey data into the patient registry system.
2. Monitors patient/physician visit schedules (IDX and Allscripts) to capture patient data at critical time periods in their care.
3. Maintains clinic flow to ensure that survey completion does not interfere with clinical care of physician visit routines.
4. Ensures follow-up of patient documentation and uses paper survey data to capture critical data when patients are unable to use the registry system.
5. Mails appropriate surveys and contact patients to collect survey data as required.

II. Position Qualifications:

License/Certification/Education:
Required:
1. Associates Degree in Health Business Administration or a related field.

Experience/Skills:
Required:
1. Three years of related experience.
2. Proficiency with Microsoft Office applications, including Access.
3. Familiarity with medical terminology.
4. Ability to manage daily work to meet department goals.

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.

III. Physical Demands and Environmental Conditions:

Work is considered sedentary. Position requires work indoors in a normal office environment.
