Our ranges are developed and published as good faith estimates for the position.*** Starting Pay Range: $22.92 - $26.32 per hour Office Benefit Package: $50/Month subject to change RADIAS Health is hiring a Housing Case Manager to join our ReEntry House Housing Support Program (HSP) in providing services to individuals experiencing long term homelessness who have mental illness and/or substance use disorders. The Housing Support program helps clients find their own independent housing within Hennepin County and provides supportive services to help them maintain their housing. The Housing Case Manager leads the housing search and provides support and advocacy to overcome barriers affecting housing in the past. They help clients with appointments for housing needs, accessing community resources and basic needs, crisis intervention, goal planning, and assists clients in obtaining Housing Support benefits. Housing Support Specialists will also provide instruction and monitoring of independent living skills to ensure clients have the supports necessary to manage as independently as possible in an apartment setting. Hours: Monday - Friday 9 am - 5 pm, 40 hours per week Location: This position works onsite at the ReEntry House HSP office located in Minneapolis Duties and Responsibilities:
- Provide instruction and monitoring on the identified service areas indicated in the Housing Focused Person Centered Plan and/or Professional Statement of Need
- Using person-centered approaches, develop a plan to get and/or keep housing
- Continually monitor client progress and modify the service plan as appropriate.
- Communicate changes in service needs and recommendations to Program Director and other people involved with the person's support.
- Maintain records for each face-to-face meeting and telephone contact with client and other providers.
- Maintain routine for visiting clients based on the amount of time and frequency agreed upon with the client, and as needed based on the client's immediate needs.
- Respond and intervene as necessary for emergencies or to address landlord concerns. Contact appropriate emergency services, staff, and contacts for additional support.
- Coordinate and cooperate with other agency and community providers to provide the best possible care for the client, including but not limited to landlords and other community resources. The Mental Health Practitioner will complete all necessary Release of Information documents for communication with other providers and keep these current for each client.
- Assist clients in obtaining and maintaining HSP benefits, and provide necessary assistance in budgeting monthly funds as required by HSP program.
- Complete all paperwork, reports, and charting contemporaneously and in an organized manner.
- Must be able to transport clients in the community.
Requirements
- One year of experience (2,000 hours) providing services to vulnerable populations (people experiencing homelessness, unstable housing and/or people with co-occurring substance use disorders and mental illnesses).
- Must have car, auto insurance, and valid driver's license
Preferred
- Bachelor's degree in the behavioral health sciences
