Position: Clinic Coordinator - CMA for Population Health

Wage Range: $50,000-$60,000 annually, depending on experience

Service Area: Lifespark Corporate Office, St. Louis Park 55416 (this is an in-person position, providing telephone and virtual care to our clients)

Schedule: 1.0 FTE, Monday - Friday 8:00am-5:00pm

The Clinic Coordinator (CMA) will split their time between direct client care in the field and leading quality improvement initiatives, with a strong emphasis on value-based care, enhancing client experience and meeting Medicare quality measures.

Position Functions & Responsibilities:

Clinical Duties (Approx. 50%)



  • Provide direct client care out in the field settings (client homes), including vital signs, screenings, and care coordination.
  • Support providers with pre-visit prep and during client visits and procedures.
  • Maintain accurate and timely documentation in the electronic medical record (EMR).
  • Educate clients on care plans, preventive care, and follow-up instructions.


Quality Improvement & Administrative Duties (Approx. 50%)



  • Lead and support quality improvement (QI) projects focused on client experience and Medicare quality metrics (e.g., HEDIS, CAHPS, STAR ratings).
  • Analyze client satisfaction data and clinical outcomes to identify trends and opportunities for improvement.
  • Collaborate with the APP Quality Lead, and Director of Initiatives Development and Delivery, and clinical teams to implement evidence-based practices and workflow enhancements.
  • Participate in audits, compliance reviews, and reporting for internal and external stakeholders.
  • Serve as a liaison between clinical staff, clients, and leadership to ensure alignment with organizational goals.


Metrics

To gauge Clinic Coordinator-CMA effectiveness, results are measured using the following indicators and targets:



  • Client satisfaction
  • Clinical staff satisfaction
  • Quality metrics



Qualifications:

Required Qualifications



  • Certified Medical Assistant (CMA) credential from AAMA.
  • 3-5 years of experience in a clinical setting, with demonstrated client care and administrative responsibilities.
  • Experience with Medicare quality programs and client experience initiatives.
  • Strong interpersonal, communication, and organizational skills.
  • Proficiency in EMR systems and Microsoft Office Suite.
  • Ability to work independently and collaboratively in a fast-paced environment.


Preferred Qualifications:



  • Experience in primary care or community health settings.
  • Familiarity with quality improvement methodologies (e.g., PDSA cycles, Lean, Six Sigma).
