Position: CNA, Certified Nurse Assistant

Location: High Pointe House

Hours: Per diem

High Pointe House is located on 39 beautiful, wooded acres in Haverhill, MA, High Pointe House provides a comfortable, welcoming home-like alternative for patients in need of specialized end-of-life care. With twenty-one private suites, our goal is to ensure the Hospice House is available to patients and families when time is most precious. One of only two inpatient hospice facilities in Massachusetts licensed to care for children as well as adults, High Pointe House address's these complex medical needs with compassion and dignity. It is a place for patients and families to spend their last days together in peace, comfort and security.

A Hospice Aide is a trained paraprofessional who provides personal care and other incidental care to patients as instructed by and under the supervision of the professional clinician. Hospice Aides provide care to patients in a variety of settings, including patients under home health and hospice plans of care, residing in private homes, assisted livings, skilled nursing facilities and hospice inpatient facilities.

Essential Functions

  • Provides care to patients as instructed in the home health aide or hospice aide care plan. Provides the level of assistance designated in the plan of care, ranging from supervision and cueing to total care.

  • Notifies the supervising clinician and or aide supervisor promptly when the patient refuses care or the patient's care needs change.

  • Provides patient care including bathing, dressing, grooming, ambulation/mobility, toileting and nutrition/hydration.

  • * When assisting with grooming and dressing, including hair styling, manicure and pedicure, supports patient and family choices for clothing and accessories, hearing aids and eyeglasses.

  • Provides bowel and bladder training, catheter care, ostomy care, and adaptive equipment (e.g., raised toilet seat) as designated in aide care plan.

  • Identifies risks to personal safety (e.g. firearms in the home, oxygen safety) and promptly reports concern to aide supervisor.

  • Follows designated infection control protocol including universal precautions, biohazardous disposal, and isolation techniques.

  • * Identifies changes in mobility, weakness, sleepiness, and fatigue and reports these changes to the supervising professional clinician

  • Identifies physical, mental and functional changes and reports these changes to the supervising professional clinician

  • Identifies respiratory concerns, such as a change in breathing pattern, or increased secretions and reports these changes to the supervising professional clinician

Position Type/Expected Hours of Work & Travel

This is a per diem position. Days and hours of work are based on operational needs with some flexibility based on position. Hours are not guaranteed and may fluctuate with the patient census.

Qualifications
  • Certified CNA or HHA certificate with ability to pass written and demonstration competencies as required by regulations upon hire. Depending on the position, the aide may be required to pass additional competencies, including Dementia Care and Hospice Care.
