If you are located in Utah, you will have the flexibility to work remotely* as you take on some tough challenges.

Primary Responsibilities:



  • Performs medication review for patients to identify medication related problems and optimization opportunities:

    • Collect relevant information from various sources to understand the history and clinical status of the patient
    • Complete patient-centered assessment, considering functional status, risk factors, cultural factors, treatment goals, and health literacy
    • Assess medications for appropriateness, effectiveness, safety, and adherence
    • Develop disease management care plan with prioritized recommendations that are evidence-based and patient-specific
    • Implement recommendations in collaboration with the patient's care team
    • Develop a follow-up plan to monitor and evaluate the effectiveness of recommendations implemented


  • May complete medication reconciliation post-discharge for high-risk patients and provide recommendations to providers and counseling to patients transitioning from hospitals, SNFs, institutions or other high-risk patients

    • Identify discrepancies between medications on hand, discharge medication list, or other medication list provided
    • Review medication regimen for drug therapy problems that may increase risk for readmission


  • Implement and support pharmacy & medical strategies and programs to achieve key strategic and business goals
  • Collaborate with functionally diverse teams (i.e. MD, RN, NP, PA-C, PharmD, LCSW) to achieve desired end-results and develop innovative solutions for complex problems. Review medication regimen for drug therapy problems that may increase risk for readmission
  • Performs all other related duties as assigned

Required Qualifications:



  • Graduation from an accredited College/School of Pharmacy with a B.S. with 10+ years of clinical experience or a Doctor of Pharmacy (PharmD)
  • Active, unrestricted Pharmacist License in applicable state(s) and willingness to get licensed in additional states as requested
  • 1+ years of Post-Graduate/Residency training or equivalent 3+ years of practice
  • Proficiency with Microsoft Office Tools and other technical applications (i.e. EMRs)
  • Proven solid prioritization, attention to detail, effective multi-tasking, and organizational skills
  • Demonstrated verbal and written communication skills
  • Proven self-motivated with critical thinking and problem-solving skills
  • Demonstrated ability to work well with others and thrives in an interdisciplinary environment

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The salary for this role will range from $89,900 to $160,600 annually based on full-time employment. We comply with all minimum wage laws as applicable.
