Summary of Job Duties:The Social Services Consultant provides supportive services to family and care team members throughout the patient's assessment and treatment. Duties may include but are not limited to, meeting with patients, conducting needs assessment, providing referrals to community services. Qualifications:

Minimum Qualifications:

  • Bachelor degree in social work, counseling community health or related field plus one year of experience in care and resource coordination for clients in medical settings.

Preferred Qualification:

  • LSW preferred

Job Requirements:

  • Conduct assessments to determine the social, emotional, and economic needs of clients or communities.
  • Identify appropriate support services and resources.
  • Develop Service Plans
  • Create individualized service or care plans tailored to the needs of clients.
  • Coordinate with multidisciplinary teams (social workers, healthcare providers, legal representatives).
  • Offer consultation to social workers, case managers, or organizations on best practices and resources.
  • Stay updated on policies, programs, and regulations that affect service delivery.
  • Evaluate the effectiveness of social service programs.
  • Recommend changes or improvements to enhance outcomes and compliance.
  • Provide training and technical assistance to staff or partner agencies.
  • Educate stakeholders on service delivery models, eligibility requirements, or documentation processes.
  • Act as a liaison between agencies, clients, and government departments.
  • Advocate for client access to housing, healthcare, food assistance, mental health services, or other support systems.
  • Performs other duties as needed.
