Work Schedule: 90% FTE, 36 hours/week. 12-hour day/evening shifts scheduled between the hours of 7:00 AM - 7:30 PM or 8:00 AM - 8:30 PM. We are looking for experienced Paramedics to join the HBHC interdisciplinary care team to provide care to hospitalized patients receiving acute care in their home, including clinical assessments, medication administration, vital sign collection, and patient education. We are seeking a Mobile Integrated Health Professional to:
Provide hands-on clinical care and acute support to hospitalized patients in their home setting Conduct clinical assessments, delivering delegated medical care, documenting interventions, and collaborating closely with physicians, nurses, and ancillary staff to ensure the safe, efficient, and patient-centered delivery of acute care at home Perform home safety assessments, adapt patient and caregiver education to unique situations, and support care through in-person visits and digital health technologies Perform interdisciplinary case conferencing and timely, thorough documentation in the electronic health record

Education
- Completion of an accredited Paramedic program Required
Work Experience
- 2 years of field paramedic experience Required
- 1 year of experience in home-based care settings Preferred
Skills and Abilities
- Experience documenting in Epic or other EHR systems
- Ability to function with a high degree of independence, flexibility, and autonomy
- Ability to collaborate with other disciplines to provide care
- Excellent interpersonal and group process skills
Licenses and Certifications
- Active Paramedic license in the state of Wisconsin Upon Hire Required
- Basic Life Support (BLS) / CPR Certification Upon Hire Required
- Advanced Cardiovascular Life Support (ACLS) Upon Hire Required
- Community Paramedic (CP) endorsement through the State of Wisconsin Department of Health Services within 12 months Required
- Community Paramedic (CP) endorsement through the State of Wisconsin Department of Health Services Upon Hire Preferred
