The Sr Medical Outreach Assistant serves as a liaison between the patient/family and the health systems providing navigation to facilitate access to care, resources and improved communication to assure patient follow-up, long-term chronic care management and reduce health needs.

Licensure, Certification, or Registration Requirements for continued employment: N/A Experience REQUIRED: Minimum of three (3) years' experience working in a health and/or community health setting Previous experience utilizing strong interpersonal, problem solving and analytical skills Excellent customer service and conflict resolution skills including the ability to handle/escalate difficult situations Experience PREFERRED: Five (5) years' experience working in community service, health or social service sector Experience working with underserved populations Education/training REQUIRED: High School Diploma or equivalent Education/training PREFERRED: Associate or Bachelor's Degree in Health Sciences or Social Services from an accredited program Independent action(s) required: Sets goals and plans to accomplish role/responsibilities. Able to handle multiple priorities and manage time to meet workload goals. Documents interactions with clients in appropriate format and software. Supervisory responsibilities (if applicable): Provides assistance with supervisory responsibilities, including performance evaluations, onboarding and training. Additional position requirements: May work all three shifts, weekends and holidays. Individual shift rotation and schedules will be established with the Manager who will determine the needs for appropriate coverage of services. Schedules, including requirements of shift rotations and hours of work, including weekends and holidays, may be adjusted as necessary to meet program and work requirements and/or as needed. Outreach services may be conducted by face-to-face contacts during home or community visits, telephone contact and/or written communication.
