The Manager of Care Management (MCM) leads and directs all care management activities, ensuring patient care coordination, adherence to clinical protocols, and regulatory standards. The MCM develops and implements care management programs, policies, and strategies in collaboration with senior leadership. This role oversees a multidisciplinary team and ensures quality, cost-effective care delivery across the continuum. The MCM reports directly to the Chief Operating Officer.

I. Major Responsibilities:

Leadership and Oversight
1. Manages case management and social services staff, including hiring, evaluations, and supervision.
2. Ensures timely and compliant patient assessments and discharge planning.
3. Provides guidance to staff and facilitates patient care conferences.
4. Oversees social work functions, including psychosocial evaluations and advocacy coordination.
5. Promotes collaboration among care management, social work, and healthcare teams.

Utilization and Quality Management
1. Oversees utilization management and ensures appropriate care settings.
2. Collaborates with physician advisors and manages ambulance service processes.
3. Implements quality improvement initiatives and monitors documentation standards.

Professional Development and Role Modeling
1. Serves as a professional role model and stays current with care management trends.
2. Participates in hospital committees and supports staff development.
3. Promotes a safe, caring environment conducive to health teaching.

Managerial Standards
1. Links departmental goals to hospital mission and improves work processes.
2. Provides staff orientation, training, and performance evaluations.
3. Communicates effectively with staff and other departments.
4. Demonstrates problem-solving and resource management skills.
5. Ensures compliance with HR policies and regulatory standards.

Age-Specific Competency
1. Applies knowledge of growth and development stages to patient care.
2. Demonstrates age-appropriate communication and support.

II. Position Qualifications:
License/Certification/Education

Required:
1. Bachelor of Science in Nursing or Master's in Social Work
2. Massachusetts RN or LICSW license
Preferred:
1. Master's in Healthcare or MBA
2. Certified Case Manager (CCM)
3. Certified Professional in Healthcare Quality (CPHQ)

Experience/Skills
Required:
1. 5+ years of inpatient case management or social work experience
2. 2+ years of direct management experience
3. Strong writing, project, and people management skills
4. Excellent communication, coaching, and conflict resolution skills
5. Ability to manage performance, prioritize tasks, and lead change
6. Strategic planning, financial oversight, and process improvement experience

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
