General Purpose of Job: Responsible for the maintenance, development, and implementation of current and future clinical pharmacy programs and services. Provide clinical pharmacy services with a focus on internal medicine, assist in the department's team member development programs, and fulfill preceptor responsibilities for pharmacy residents, pharmacy students and medical students. |

- Provide comprehensive medication management for adult internal medicine patients with complex and chronic conditions, ensuring safe, effective, and evidence-based pharmacotherapy.
- Collaborate with physicians, nurses, and other healthcare professionals to optimize therapeutic outcomes and support transitions of care within the inpatient internal medicine setting.
- Perform clinical pharmacokinetic dosing service in a timely and accurate manner.
- Provide drug information services.
- Regularly interface with individual medical services and improve the quality of pharmacy services to those patients.
- Participate in Pharmacy and Therapeutics Committee activities.
- Participate in research and professional communications.
- Assist in the department's team member development programs by presenting and coordinating Pharmacy seminars, educating and training pharmacy residents and pharmacists in clinical programs, and serve as a resource on pharmacotherapy.
- Write clear objectives for program development with target dates outlined and attend clinical staff meetings.
- Audit and assure pharmacy residents and quality of existing clinical programs.
- Assist in the training, instruction, supervision, and evaluation of pharmacy students and pharmacy residents assigned to this site, and participate in department orientation.
- Perform duties of a staff pharmacist on assigned schedule.
- Compounding and supervising the compounding of Non-sterile medications up to and including Moderate Compounding as defined by USP<795> and Sterile medications up to and includingMedium Risk as defined by USP <797>
- Handle, dispense, verify, and manipulate hazardous medications as defined by USP<800> to meet patient care needs.
- Compounding team members adhere to the prescription or medical order for the strength, quality and purity of compounding preparations. Compounded products are packaged, labeled and dispensed according to current laws and regulations.

Job Requirements
General Requirements |
* Valid State or Michigan Pharmacy license in good standing with the State of Michigan required. * Valid State of Michigan Controlled Substance license in good standing with the State of Michigan required. * Completed a Pharmacy Residency or maintain certification from The Board of Pharmacy Specialties |
Work Experience |
* One year hospital experience preferred. |
Education |
* Graduate of an accredited College of Pharmacy. * Doctorate of Pharmacy Degree |
Specialized Knowledge and Skills |
* Demonstrates effective oral, written and phone communication skills * Demonstrates ability to use a keyboard as may be required to perform the essential duties of the job. |
