Prevention Nurse, Prevention & Wellness

The Prevention Nurse is responsible for identifying, documenting, and troubleshooting barriers to patient care. This individual reviews abnormal screening test results with the Nurse Practitioner or Physician and obtains orders for appropriate patient navigation and care. This individual must be familiar with all specialty groups and service lines within Norton Healthcare and Norton Cancer Institute. The nurse is responsible for referring eligible patients to financial assistance both within and outside Norton Healthcare when appropriate. This individual plans, organizes, and implements community outreach activities while working closely with the Marketing department, and delivers direct education, patient screenings and coaching at these scheduled events. The Prevention Nurse facilitates and educates the nursing staff and monitors the nursing care delivered to ensure compliance with the Norton Healthcare and Center for Prevention and Wellness (CPW) guidelines and policies.

Required:

• Three years nursing and/or patient education
• Diploma
• Registered Nurse (KY)

Desired:

• One year wellness and health promotion, public health and/or community health, Ability to speak Spanish.
• Bachelor Degree
