RN - Specialty Nurse Navigator - Vascular - La Jolla

This is a Full time, benefited RN Vascular Nurse Navigator position that starts between 8-9am and is located in La Jolla. Some on call and weekend shifts may be needed at times.

Job duties include: Formulates custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care. Establishes specific, measurable, attainable, relevant and time specific nursing goals to guide the care planning process and promote self-reliance. Assists patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance. Identifies and aligns resources that impact high-risk care. Coordinates services on behalf of the high-risk patients. Work in collaboration with the Healthcare team to track and review data for research projects as well as resource utilization related to relevant specialty and make recommendations as needed. Schedules patients and coordinates cases with the doctor and other departments. Prepares paperwork for the doctor to sign off on. At times assists the surgeon in the operating room.

Required Education/Experience/Specialized Skills: BSN 3 years recent clinical nursing experience in a variety of clinical settings;

Required Certification/Registration: BLS and. ACLS through the AHA. CA RN License

Preferred Education/Experience/Specialized Skills/Certification: MSN, Mechanical Circulatory Support- Two years experience managing care of relevant specialized patient population. Preferred experience with vascular care
