Job Summary

Milwaukee Public Schools seeks exceptional Instructional Coach - Transition Support candidates who are highly qualified and knowledgeable to join our community of teachers, learners, and leaders.

The Instructional Coach - Transition Support is responsible for:

  • Facilitating and assessing data for the coordinated set of activities designed, within a results-oriented process.
  • Facilitating the academic and functional achievement of students with disabilities within that process.
  • Facilitating the students' movement from school to post school activities (post-secondary education, adult services, independent living, community participation).
  • Ongoing student needs assessment, accounting for students' strengths, preferences and interests.

Qualifications

  • A current lifetime license in secondary special education (1801, 1810, 1811 or 1830) issued by the Wisconsin Department of Public Instruction.
  • A minimum of five (5) years teaching experience as fully certified special education teacher.
  • A minimum of five (5) years hands-on experience working with special education students in vocational settings or equivalent.
  • Experience with high school special education students preferred.
Responsibilities

Provides staff training in the development of effective Transition Plans, Summary of Performance and Notice of Graduation.

Disseminates transition resources and related information to school staff

Supports staff to:

  • Learn about age-appropriate transition assessments
  • Collaborate with Employment Training Specialists to identify students who are on track to graduate and those students who may need additional transition services.
  • Facilitate connections between families/students and adult agencies
  • Facilitate appropriate referrals to school and district based and community programs

Works with school staff to provide effective transition planning

Supports School to Work Program to:

  • Facilitate and coordinate referral process
  • Organize and support transportation
  • Visit work sites
  • Monitor/provide assistance to students' performance at work sites and communicate results to staff, students and parents
  • Act as IEP case manager for students who are enrolled in Project Search

Provides support to students who are employed

Attends IEP meetings

Acts as a liaison between DVR and case managers

Plans family engagement opportunities in transition planning to support district, school and student goals

Maintains records

Provides leadership in transition planning

Attends required meetings

Submits monthly reports

Middle School Support/Responsibilities

  • Supports Special Education Staff in transition planning
  • Trains and supports Special Education Staff with Postsecondary Transition Plans (PTP's)
  • Shares transition assessments/resources with Special Education Staff
  • Works with Special Education Supervisors to plan one department meeting per semester with a focus on transition

This position is on the 200-day calendar.
