Case Manager - Culinary Lead

AllHealth Network is currently looking for a qualified Case Manager who will also oversee our teaching kitchen, focused on teaching clients culinary and kitchen safety skills. As a team we strive every day to nurture growth and recovery by caring for each other, our clients, and our future.

Shift:

Full-Time: Monday - Friday

Pay Rate:

$23 - $25

The base salary range represents the low and high end of the AllHealth Network salary range for this position. Actual salaries will vary and may be above or below the range based on various factors including but not limited to experience, education, training, merit, and the ability to embody the AllHealth Network mission and values. The range listed is just one component of AllHealth Networks' total compensation package for employees. Other rewards may include short-term and long-term incentives as well as a generous benefits package detailed below.

Job Description:

Case Manager - Culinary Lead



  • Case Manager Duties:

    • 50% case manager for outpatient including 1:1 appointments for benefit acquisition and resource navigation
    • 50% drop-in services for clients
    • Utilize key concepts of recovery including client choice, empowerment; hope, creating meaningful activity, advocacy, and consultation with key supports, to facilitate client centered care that will help individuals achieve their personal goals.
    • Maintain a caseload of clients in accordance with department policy, to gain optimal functioning and sustain improvements in health management.
    • Complete all required care planning and assessments to guide care delivery and meet contractual obligations
    • Administer outcome and assessment tools to assist with identifying client needs, progress through treatment and development of care plans.
    • Utilize professional judgment and critical thinking to assist the client in overcoming barriers to goal achievement. Update care plans as needs change.
    • Act as a client advocate and a liaison between internal and external providers, and community resources to seamlessly integrate and coordinate services.
    • Make referrals to community-based organizations and resources that are consistent with the identified needs in the plan of care and/or by the client


  • Culinary Lead Duties:

    • Develop a milieu environment that provides for the safety of those involved in the program.
    • Through the kitchen program, assisting clients in developing menus and meals as well as meal planning on a budget
    • Oversees grocery shopping and stocking the kitchen with necessary supplies
    • Oversight of the catering program, including coordination of ordering, meal planning, grocery shopping, billing, and set-up of meals.
    • Participate in community events and social recreation programs as indicated.




Qualifications:



  • Bachelor's degree in a health/behavioral health related area of focus such as social work, psychology, sociology; or bachelor's degree in unrelated field with at least one year of experience in the mental health field
  • AllHealth Network does not employ former or current clients or family members of clients receiving treatment at any AllHealth Network facilities


  • Minimum of 2 years' experience in a behavioral health or human services setting. Familiarity with recovery and trauma informed care.
  • Experience in a culinary environment.
  • Experience in case management, care coordination, and/or community health required
  • Must be willing to obtain kitchen manager certification paid by AllHealth Network


Skills/Knowledge:



  • Demonstrated ability for assessment, evaluation, and interpretation of behavioral health information, and care planning
  • Experience working in a commercial kitchen, running a cash register, and ability to teach cooking skills
  • Excellent interpersonal skills, the ability to work independently and as part of a team; collaborating with other internal and external teams.
  • Must possess own transportation, have a good driving record, a valid driver's license and proof of insurance.
