Provides nursing support to the physician and provides direct nursing care to patients in an inpatient setting.
MINIMUM QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Current Licensed Practical Nurse license issued by the state in which services will be provided or current multi-state Licensed Practical Nurse license through the enhanced Nurse Licensure Compact (eNLC). 4. Successfully pass WV DHHR Protected Services Record Check and WV CARES fingerprint-based criminal background check. PREFERRED QUALIFICATIONS: EXPERIENCE: 1. One (1) year clinical experience. CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. Provides direct patient care, as outlined in the patient's plan of care and/or as directed by the overseeing role , to promote comfort and/or return to functional health for the patient. Consults with the Registered Nurse or Respiratory Therapist to seek guidance in delivery of nursing care, as necessary. 12. Assumes charge of the Long Term Care unit, assignes and delegates tasks to CNAs. 13. Long Term Care LPNs are expected to demonstrate adaptability and resilience in managing higher patient loads commonly found in LTC environments. 14. LTC LPN should possess strong time management and prioritization skills to effectively care for a larger number of patients, as is often required in long term care environments. 15. Independence in care delivery, rounds with the doctor. 16. Reconciliation and entry of orders. 17. Responsible for extensive documentation, complete admission process. 18. Communications with families about residents' condition. 19. Coordinates follow-up appointments. 20. Coordinates with rehab and other disciplines: PT, OT, Speech, Dietary. 21. Responsible for respiratory treatments including spontaneous aerosols, inhalers. 22. Responsible for medication distribution for 15-30 residents. For facilities with specialty accreditation requirements: 1. Completion of annual required education related to specialty accreditation as defined by accreditation standards. Training may be completed through CBLs, trainings, In-services, and competency validation. PHYSICAL REQUIREMENTS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Heavy/Hard work: Work requires strength and/or stamina, lifting, moving, stooping, reaching, positioning of patients, standing, walking; carrying of patients, materials and equipment weighing 40+lbs.
