Diabetes Program Coordinator

The RN Diabetes Program Coordinator works collaboratively with the multidisciplinary health care team to optimize care and clinical outcomes of inpatients with diabetes and/or hyperglycemia. In partnership with the Diabetes Excellence in Care Program Medical Director, the Coordinator promotes evidence -based, person-centered care through use of guidelines and order sets. In addition to direct patient care, the Coordinator also participates in program development, including, but not limited to, staff education, patient and family education and support, and community education. • 1. Collaborates with physicians and staff to ensure compliance with registry specifications and center of excellence specifications: Manages accreditation criteria to ensure program meets expectations set forth by governing agencies. Maintains and ensures compliance of all policies and procedures that guide and support the assigned services. These include, but are not limited to Joint Commission requirements, federal and state regulations, MBS-AQIP standards, regulatory standards, etc. Collaborates on an ongoing basis with Medical Directors and Division Chief, program managers, providers, clinic staff and physicians, promoting positive communication techniques and the ability to problem solve at every level. Works with Medical Directors and Division Chief to anticipate and identify the needs of the program center and creatively implements solutions to problems. Integrates Evidence Based Practice into clinic and program hospital practice improvements. Manage patients' status to ensure patients move through the process from seminar to surgery as efficiently as possible. Be knowledgeable of insurance requirements for authorization for all payors, and facilitate preauthorization process.
• 1.1 Accountable for the delivery of quality patient care utilizing a therapeutic process based on clinical practice guidelines. 1.1. Conducts individual exams, evaluation and treatment to patients including glycemic stewardship to support glycemic management during admission. 1.2 Reassesses the needs of the patients at regular intervals to provide necessary support. 1.3 Coordinates care of patients through appropriate collaboration with physicians, nursing staff and ancillary departments. 1.4 Acts as a resource to staff and physicians.

EDUCATION

RN

EXPERIENCE

education- At least 2 years of clinical experience within the past five (5) years in a hospital, out-patient program or medical practice which included the care and education of patients with diabetes and education of health care providers.

LICENSURE

License- Professional credentials: RN with current CT license Certified Diabetes Educator (CDE) by the National Certification Board for Diabetes Educators (NCBDE) and/or Board Certified-Advanced Diabetes Management (BC-ADM) preferred

SPECIAL SKILLS

Special skills- Excellent verbal and written skills/communication; ability to motivate and teach across a spectrum of positions , clinical proficiency, strong coordination, evaluation and clinical judgment skills; ability to juggle multiple priorities , ability to relate competently to all age groups and to patients with varied socioeconomic and ethnic backgrounds . Computer skills required. Understanding of federal, state and TJC standards and regulations. Patient Educator Knowledge of the preoperative multidisciplinary preoperative preparation of the patient Ability to develop written materials for patient education Knowledge of the types of bariatric surgery; risks and complications of each and expectations of each procedure. Minimum Knowledge, Skills and Abilities Required: Possession of a current license as a registered nurse Minimum of 3 years registered nurse experience. Three years of current experience in the area of bariatrics preferred. BLS for the Healthcare Provider certified Elevated level of interpersonal/communication skills to promote team approach. Excellent written and verbal skills required.
