Social Services Manager Long Term Care

About the facility

With 2 floors, 4 units, we are able to provide care for long term care residents and post-acute rehab patients. The first floor has 96 beds and the second floor has 100 beds. Nurse-patient ratios vary depending on the shift and the population you will be serving. We care for dementia residents, long term care and a rehab residents. With 196 beds, we accept but not limited to patient specialties, such as general rehab, neuro rehab, cardiac, respiratory and cancer. We also have various programs for residents to participate in

multiple activities.

Job Summary

The Social Worker provides and/or coordinates medically related social services to attain and maintain the highest practicable physical, mental and psychosocial well being of each resident. This activity is achieved through the use of appropriate assessment tools, development, completion and revision of resident care plans and maintaining contact with residents and their family members. The Social Worker ensures that all charted progress notes are informative and descriptive of the services provided, including the resident's response to the service.

Qualifications

  • Required Bachelor's Degree Social Work: Exceptions for persons without a social work degree can be made for those who have a human services degree and carry a current social work license.
  • 1 year of relevant experience Supervised social work experience in a health care setting working directly with individuals. Preferred
  • 1 year of relevant experience Skilled nursing home process and procedure Preferred
  • 1 year of relevant experience Community resources and discharge planning Preferred
  • 3 years of relevant experience Hospital or Nursing home social work Preferred
  • LIC-Bachelor Social Worker (BSW) - STATE_MI State of Michigan Upon Hire required
