The medical social worker is responsible for the implementation of standards of care for medical social work services for hospice and home health.
ESSENTIAL RESPONSIBILITIES:
- Assess the psycho-social status of patients related to the patient's illness and environment and communicate findings to the RN Case Manager.
- Carries out social evaluations and plans interventions based on evaluation findings.
- Maintains clinical records on all patients referred to social work.
- Provides information and referral services for patients and families/caregivers regarding practical and environmental needs.
- Maintains collaborative relationships with organization personnel to support patient care.
- Participates in the development of the total plan of care and case conferences as required.
- Participates in discharge planning.
- Conducts meetings with perspective patients/families to review philosophy of hospice and the hospice benefit.
- Assist with bereavement process as needed.
EDUCATION AND WORK EXPERIENCE: Required Degree: Master's or doctoral degree in social work. Certificate(s): CPR KNOWLEDGE, SKILLS, AND ABILITY:
- Ability to communicate (written and verbally) in English with patients, families and staff.
- One year health care experience. Home Health or Hospice experience preferred.
- Must have a reliable means of transportation.

- Understand and follow all regulatory and corporate policies related to confidentiality of the resident's medical, financial and personal information.

PHYSICAL REQUIREMENTS:
- While performing the duties of this job, the employee may be regularly required to stand, sit, talk, hear, reach, stoop, kneel, and use hands and fingers to operate a computer, telephone, and keyboard.
- Specific vision abilities required by this job include close vision requirements due to computer work.
- Light to moderate lifting may be required.
- Regular, predictable attendance is required.
$26.54 - $35.00
Reasonable Pay Estimate A reasonable estimate of the pay range for this position is $26.54 - $35.00. There are numerous factors taken into consideration in determining the actual offered rate of pay, including but not limited to: job-related qualifications, experience, skills, education, geographic location, and consideration of internal and external equity.
