Healthy Opportunities (HOP) Eligibility & Enrollments, Benefits Eligibility Screenings, Cold Calls, Assistance in phone queue

1.	Proficient with excel/word	1.
2.	Experienced MSW/SW in a healthcare/community setting	2.
3.	Community work, experience with closing SDoH needs	3.
