Summary

The Admissions Coordinator oversees the entire admission process. This employee is responsible for insurance verification, room assignment and all admission paperwork. The Admissions Coordinator will maintain all statistical data on referrals, denials and admissions.



Responsibilities

  • People - Pre-admission phase - receive referrals via telephone/fax; contact PCR with evaluation and communicate this with case manager from host hospital; verify and print out Medicare HIQA form on a daily basis..
  • Verification of bed/room status - communicate with medical staff to determine appropriate room available to meet the specific needs of the patient (i.e. male, female, isolation bed, vent, etc.) on a daily basis.
  • Coordination of specialized equipment and services - communicate with medical staff in order to obtain necessary equipment/dme and particular services to meet the specialized needs of the patient on a daily basis. For example, clinitron beds, specific therapy dme, specialty mattress covers, dialysis center, etc.
  • Scheduling of transportation - communicate with community agencies to provide necessary transportation in order to transfer patient from one facility to the extended care hospital. A specific date and time will be obtained prior to admit (schedule with case manager from host hospital) on a daily basis.
  • Notification of patient admit - notify all medical staff of admit, room number, and approximate time of arrival; make copies of evaluation form for appropriate staff; obtain updated patient information to provide to charge nurse if applicable. If a patient is not approved for admission, will notify referring facility (i.e. case manager) on a daily basis.
  • Enter patient admit into HCS system - follow the appropriate guidelines and generate a patient face sheet for the chart on a daily basis.
  • Patient/family orientation - meet with patient and/or family members to complete the hospital admission packet and all forms will be discussed and processed at that time on a daily basis.
  • Maintain daily log on all referrals (both admits and non-admits) - report to Social Worker on quarterly basis.
  • Maintain and coordinate patient room assignments - Using HCS system, enter any room swaps and/or room transfers on daily basis and notify medical staff and patients' families of any changes; monitor and enter any interrupted stays for in-house patients; discharge patients from the system appropriate to date/time/location on a daily basis.
  • Ensure all rooms are cleaned after discharge in a prompt manner.
  • Assist with coordinating and maintaining supplies, if necessary
  • Assist the DON, Office Manager, and Medical Staff with related tasks necessary to the overall management and maintenance of the unit on a daily basis.



Education and Experience

  • Formal Education: High School Diploma or equivalent.
  • High school diploma or equivalent.
  • Must possess exceptional organizational, time management, communication and telephone skills.
  • Must have basic computer knowledge and ability to work in HCS, Excel and Microsoft Word.
