***This is a time limited grant funded position with anticipated assignment period lasting until 9/30/2027 with a set pay rate of $32 per hour ($66,560 annually).***

LOCATION: Remote - this is a home-based, virtual position that operates Monday - Friday from 8:30am - 5:00pm EST. Incumbent must live in North Carolina or within 40 miles of the North Carolina border. Must be willing to travel to the Asheville NC office if needed.

GENERAL STATEMENT OF JOB

The SPARK Transitions of Care Specialist serves as a member of the Vaya Provider Network Operations team and works closely with the SPARK participant's team to ensure continuity of services and supports during all phases of Competitive Integrated Employment (CIE). The SPARK Transitions of Care Specialist partners closely with the SPARK Navigator to coordinate the delivery of services for SPARK program participants and includes comprehensive assessment of individual needs in the areas of housing, day activity/employment, physical health, income/finance, psychological/psychiatric treatment and others. The assessment is a compilation of developmental, educational, vocational, social and psychological evaluations as well as information from the person's family and other service providers. The SPARK Transitions of Care Specialist provides support to the SPARK provider who develops a service plan for the individual and his/her team using a person-centered planning process. This process includes the strategies for meeting the needs of the individual and obtaining available services of their choice. The SPARK Transitions of Care Specialist then support the SPARK provider the individual and family by linking or connecting them with the identified, available service provider(s) or resources.

This position will also assist with Provider Transitions of Care related to provider suspensions, terminations, or non-renewals. This will involve working closely with providers and internal Vaya departments to ensure a successful and smooth transition of care for Vaya members.

ESSENTIAL JOB FUNCTIONS

SPARK Coordination activities:

  • Support the completion of a comprehensive assessment of individual needs once identified by the SPRK Navigator
  • Support the provider to develop of a service plan identifying services, providers, potential dates of service, and funding
  • Support the provider to Link, advocate, and monitor service delivery and adjust plans as needed for ongoing success
  • Maintain close partnership with potential service providers and provide necessary training on service delivery, billing, and expectations
  • Provide internal advocacy to ensure effective provider network and adequate funding CIE.
  • Provide Internal advocacy and education to Vaya staff around the benefits of CIE
  • Attend internal and external meetings involving the SPARK program

Provider Transition of Care:

  • Manage and oversee the execution of Provider Transitions of Care relating to all provider termination scenarios including provider termination and DHSR Sudden Closure notice intake, contract manager assignments, status/progress tracking and internal status reporting.
  • Manage the Provider Enrollment File (PEF) Termination Outreach tracker and coordinate with PN team to ensure timely research, outreach and termination processing according to Vaya's contract with DHHS.
  • Manage and oversee the Provider Transition of Care Tracker and coordinate with the PN team to ensure required timeframes are met and captured.
  • Assist with processing of Provider terminations and transitions of care using Vaya's Provider Transition of Care Policy and the established Provider Network process.
  • Outreach providers for status updates regarding transitions of care.
  • Coordinate within Provider Network Department and across Vaya Departments to ensure that members receive required notification and coordination of care during transitions related to Provider contract terminations.

Other duties as assigned.

KNOWLEDGE, SKILLS, & ABILITIES

Candidates for this role should have a strong operational knowledge of the Division of Employment and Independence for People with Disabilities (formerly Vocational Rehabilitation) and Medicaid and state funded employment services authorized through Tailored Plans.

QUALIFICATIONS & CREDENTIALING REQUIREMENTS

Bachelor's degree and 5 years of experience in MH/SU/IDD services.

Preferred experience:

Qualified Professional (QP) with experience in Care Management and/or Employment Services for individuals with MH/SU/IDD.

PHYSICAL REQUIREMENTS

  • Close visual acuity to perform activities such as preparation and analysis of documents; viewing a computer terminal; and extensive reading.
  • Physical activity in this position includes crouching, reaching, walking, talking, hearing and repetitive motion of hands, wrists and fingers.
  • Sedentary work with lifting requirements up to 10 pounds, sitting for extended periods of time.
  • Mental concentration is required in all aspects of work.


RESIDENCY REQUIREMENTS: The person in this position is required to reside in North Carolina or within 40 miles of the NC border.

SALARY: Depending on qualifications & experience of candidate. This position is non-exempt and is eligible for overtime compensation.
