As Case Manager Registered Nurse, you will participate with the outpatient team of professionals to meet discharge goals of patient and families, monitor outcomes and utilization of services, maintain communication with payor representatives and provides age appropriate clinical intervention to meet psycho-social and referral needs of identified patients and families. Position: Case Manager, Registered Nurse
Shift: Days, Per Diem Hours

Experience:
1. Minimum of two years recent experience in a hospital or rehabilitation facility required.
2. Minimum of two years recent insurance, case management experience preferred.

Education:
1. MSW or RN required.
2. CCM preferred.
