Responsibilities

The RN Nurse Navigator serves as a critical liaison between patients, families, and the healthcare team, providing comprehensive support throughout the patient care continuum. This role is designed to enhance patient experiences, improve health outcomes, and streamline care processes, ensuring patients receive timely and appropriate interventions.

Key Responsibilities:

  1. Patient Advocacy:

    • Act as a patient advocate, guiding individuals through the healthcare system.
    • Provide education on treatment options, resources, and available services.
  2. Care Coordination:

    • Coordinate patient care across multiple disciplines and settings.
    • Facilitate communication between patients, families, and healthcare providers.
  3. Assessment and Planning:

    • Conduct comprehensive assessments to identify patient needs.
    • Develop individualized care plans in collaboration with the healthcare team.
  4. Education and Support:

    • Educate patients and families on disease processes, treatment plans, and self-management strategies.
    • Offer emotional support and counseling to patients and their families.
  5. Follow-Up Care:

    • Monitor patient progress and adherence to treatment plans.
    • Schedule follow-up appointments and ensure continuity of care.
  6. Data Management:

    • Maintain accurate records of patient interactions and care plans.
    • Utilize data to track patient outcomes and identify areas for improvement.
  7. Quality Improvement:

    • Participate in quality improvement initiatives and patient safety programs.
    • Collaborate with the healthcare team to enhance patient care processes.

Qualifications
  • Education:

    • Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.
  • Licensure:

    • Current Registered Nurse (RN) license in Illinois
