Position Overview: Reporting and accountable to the Manager of Health Information Management Operations; facilitates medical record documentation completion and release of records from the Northwest Community Healthcare (NCH) Electronic medical record and is responsible for quality control of imaged medical record documents. Has accountability for data integrity of legal medical records across the NCH continuum. Works closely with the medical staff members to ensure that medical records have the required documentation in the required time frame.
What you will do:
- Analyzes, and technically evaluates inpatient and outpatient medical records for completeness by ensuring all required reports have been documented correctly and contains the required elements which meet Joint Commission and NCH requirements. Maintains work-queues for deficiency analysis, verification, and initial analysis, within the Epic system.
- Ensures the MPI uniquely identifies each patient who receives services at NCH. Identifies, researches and resolves duplicate or potentially duplicate patient records. Performs necessary computer functions to correct patient data within the MPI, including additions, deletions, merges and revisions in current and all legacy EMRs. Ensures merges have been done in all appropriate databases including communication to ancillary departments.
- Monitors chart completion/delinquent process; submits timely and accurate notification to physicians of their number of delinquent records. Generates various reports throughout the month to monitor the number of incomplete/delinquent records based on Joint Commission standards and NCH Rules and Regulations.
- Ensures data integrity by performing 100% quality review of imaged documents to every page scanned from the actual medical record. Reviews images that were scanned for quality control purposes and identifies pages that did not scan well. Scans loose documents and indexes them to the patient as needed. Re-scans documents if necessary.
- Provides assistance to physicians with various requests for information and assistance with access to electronic records. Communicates effectively with physicians and physician offices who have questions and concerns regarding their deficient records.
- Performs Health Information Management Technician I job duties when needed based on workflow volumes and staffing shortages.
- Works on special projects as requested by the Manager or Director.
- Adheres to all Northwest Community Hospital standards, policies and procedures.
What you will need:
- Education: High School Diploma
- Experience: Minimum one year medical record experience in a healthcare environment
- Skills: Knowledge of medical terminology, anatomy and physiology. Knowledge of Joint Commission standards and federal and state regulations required. Knowledge of and experience with health information computer systems required. Previous use of electronic imaging and medical record systems preferred.
