Below, you'll find other important information about this position. We are seeking a dedicated and experienced Home Health and Hospice Educator to join our team. This role is pivotal in ensuring the highest standards of care through comprehensive onboarding, mentorship, and continuous education of our home health and hospice staff. The ideal candidate will be passionate about fostering professional growth, ensuring regulatory compliance, and enhancing clinical competencies.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Current RN licensure.

2. Valid driver's license.

3. CPR instructor certification within 3 months of hire.

4. OASIS certified within 1 year.

EXPERIENCE:

1. Minimum of 5 years of Home Care or Hospice Nursing experience.

2. Proven experience in education, training, and staff development.

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. 1. Onboarding:

  • Develop and implement a structured onboarding program for new hires.
  • Ensure new staff are well-integrated into the team and understand organizational policies and procedures.
  • Review the orientation checklist weekly with new employees and sign-off activities/skills once complete.
  • Complete onboarding competencies with new employees.
  • Accountable for the entire staff orientation process.

2. Mentorship:

  • Provide ongoing mentorship and support to staff, fostering a culture of continuous learning and professional development.
  • Develop and support formal mentorship program.
  • Serve as a source of support and encouragement, answer questions, and provide guidance to new employees.
  • Facilitate peer-to-peer learning opportunities and encourage knowledge sharing.
  • Serve a vital role in counseling during times of work stress, maintaining confidentiality.
  • Assist in individualized IEP sessions for those associates who fall below competency levels for their job description.

3. Retention:

  • Implement strategies to enhance staff retention and job satisfaction.
  • Conduct regular check-ins and feedback sessions to address concerns and support career growth.
  • Monitor site turnover and retention rates.

4. Clinical Continuing Education:

  • Design, deliver, and evaluate clinical education programs to keep staff updated on the latest best practices and advancements in home health and hospice care.
  • Coordinate with external educators and organizations for specialized training sessions.
  • Develop and supervise systems for assessing and prioritizing educational needs.
  • Collaborate with healthcare providers in planning, implementing, and evaluating patient education to reduce readmissions.
  • Monitor patient education documentation to improve organizational performance.
  • Implement required educational training programs (e.g., defensive driving every 2 years).

5. Regulatory Education:

  • Ensure all staff are knowledgeable about current regulations and compliance requirements.
  • Conduct regular training sessions on regulatory updates and organizational policies.
  • Develop educational programs to meet Infection Control regulations for home care.
  • Collaborate with the Infection Control Practitioners for current standards.

6. Field Ride-Along:

  • Accompany staff on visits to provide hands-on training and support.
  • Assess and provide documented feedback on clinical practices and patient interactions.
  • Utilize preceptors to develop meaningful field experiences based on patient complexity and diagnosis.

7. Competency Assessment:

  • Conduct regular competency assessments to ensure staff meet clinical and regulatory standards.
  • Develop individualized improvement plans and provide targeted training as needed.
  • Review documentation and provide constructive feedback on areas needing improvement or additional clarification.
  • Ensure associates have completed orientation requirements and are competent to provide quality care.

8. Leadership and Teamwork:

  • Foster a positive and mentoring work environment.
  • Communicate effectively with all members of the interdisciplinary team.
  • Develop and maintain good interpersonal relationships with medical staff, department managers, associates, and administration for planning and implementation of programs as part of the strategic plan.

9. Educational Resource Management:

  • Coordinate, develop, and distribute current information on educational resources available by TEAMs newsletter.
  • Collaborate with key strategic hospital partners to align with patient education materials as connected to the strategic plan for business development and readmission reduction.
