Overview

Job Summary

Coordinates the process for 1) screening, brief intervention and referral to therapy (SBIRT) 2) the mental health screening and referral process for hospital admitted trauma patients and 3) support for the hospital Trauma Survivor's Network program. Provides education and support services to trauma patients, families and staff. Also provides psychosocial assessment, counseling, crisis intervention and information/referral services on a consultation basis to patients and families. Works collaboratively with the trauma team and other departments to effect positive patient and family outcomes and to ensure efficient utilization of resources.

Essential Functions

  • Oversees and coordinates the trauma SBIRT process for hospital admitted trauma patients (>12 years old) with alcohol misuse disorders.
  • Oversees and coordinates the screening and referral process for hospital admitted trauma patients at high risk for psychological sequelae.
  • Coordinates activities and planning to support the hospital's Trauma Survivors Network program.
  • For designated trauma patients:
  • Interviews patient/patient's social support system to complete initiate psychosocial evaluation.
  • Works collaboratively with the patients and patient's social support system to establish and implement a patient-centered, plan of care with multidisciplinary team that meets the patient's identified psychosocial needs.
  • Works with the trauma and Clinical Care Management team to assess and evaluate the admitted trauma patient's social support system for the appropriate level of care or environment setting to meet care needs across the continuum.
  • Completes psychosocial assessments for the patient's post hospital care for designated patients and their support system.
  • Provides information, education to patients and their social support on community resources and options for post hospital care appropriate to the age of the patients served, in collaboration with the Clinical Care Management team. Make referrals to community agencies as needed.
  • Identifies and records clinical, psychosocial and financial barriers to a smooth transition across the healthcare continuum and assists in identifying and facilitating system improvements.

Education, Training, and Experience

Master's degree in Social Work from a school accredited by the Council on Social Work Education.

A minimum of two (2) years of recent experience in medical social work.

Must be currently licensed as a clinical social worker (LCSW) in the state of Georgia.
