Description Medical Group PracticesProfessional - Clinical ~ 40 Hours/week, Full-time, M-F, Day shift ~ Hybrid (up to 2 days/week remote after training is completed) Maine State License Licensed Social Worker (LSW) is required This position will support our Portland Family Medicine practice at 272 Congress St., Portland, ME. We have an exciting opportunity for a Social Worker (LSW) to join Lend a Hand, part of our growing care management program! Our SWs provide complex care management in the primary care setting. Through sensitivity to social drivers of health, understanding of the role behavioral health challenges often play in a patient's overall health, and deep knowledge of socioeconomic safety net systems and resources, the Social Worker helps patients overcome barriers and make meaningful progress toward health-related goals. The Social Worker - MHMG role provides professional social work services that support the goal of the Population Health program to deliver risk-based population management services to patients at all times during their tenure of care.
