Job Title: Resource Specialist

Job Location: Remote, Columbus/Central region area highly preferred

Job Duration: 1+ Month

Walk me through the day to day responsibilities of this the role and a description of the project (Outside of Workday JD): Perform member outreach to resolve social determinant of health needs. CHW certification required. Customer service skills, motivational interviewing skills, ability to use computer system, electronic health record experience preferred. Closed loop rate percentage; opening assessments and resolving members needs, working through assigned tasks daily.
